An Appraisal Form is a template for users to input performance appraisal records for each employee.

Before using the performance appraisal module, at least 1 appraisal form must be created and defined. Let’s see what components are included in an appraisal form in our system.

There are 3 main components : Form, Item, Rank in Performance Appraisal module, you have to set them up first before entering actual appraisal content for each employee. See below table :

Form	First of all you may create multiple Form for different group of staffs	“Form A” for Grade A staffs, “Form B” for Grade B staffs	Many Forms can be created but only 1 Form can be selected in 1 appraisal record	Appraisal Form Setup
