What is Appraisal Form

An Appraisal Form is a template for users to input performance appraisal records for each employee.

 

Before using the performance appraisal module, at least 1 appraisal form must be created and defined. Let’s see what components are included in an appraisal form in our system.

 

 

There are 3 main components : Form, Item, Rank in Performance Appraisal module, you have to set them up first before entering actual appraisal content for each employee. See below table :

Form

First of all you may create multiple Form for different group of staffs

“Form A” for Grade A staffs, “Form B” for Grade B staffs

Many Forms can be created but only 1 Form can be selected in 1 appraisal record

Appraisal Form Setup

 

Item

Then, for each form, you may create multiple Items with Name, Description and score weighting

“Leadership”, “Problem solving”, “Quality of work”, etc

Many Items can be created but each Item can be applied to 1 Form only

Appraisal Item Setup

 

Rank

Finally, also for each Form, create multiple Ranking with name and score

“Excellent 5 points”, “Average 3 points”,  “Poor 1 point”, etc

Many Ranks can be created but each Rank can be applied to 1 Form only
